Register as a patient

If you wish to register as a patient of Clanrye Medical Practice you can collect an application form, in person, here at the surgery.

Patients wishing to be registered in our practice must have a home address within a radius of 6miles from the practice, within Northern Ireland.

The completed form, along with 2 photocopied proofs of address and 1 photographic proof of Identification will be accepted from patients wishing to register strictly between 10.00am and 11.00am on Wednesday mornings. 

We ask that you also provide a brief summary from your previous GP on registration. Once you are registered we will apply for your full medical notes to be passed to us from your previous GP.

 

Make an appointment

 

To arrange an appointment  with a doctor or a member of the multidisciplinary team please contact the surgery between 8.30am and 11am (10am on Wednesday). Please note if the surgery is exceptionally busy on occasions all available appointments may be filled prior to 11am. In this instance staff will advise you to call again on another day.

When you contact the surgery staff will ask you for some basic details to allow your call to be dealt with appropriately.

A member of our medical team will then contact you by telephone. Face-to-face appointments or home visits can be arranged as necessary after your telephone assessment.

Please phone the appointment line after 11am for emergencies only.

To book an appointment in one of our practice nurse clinics speak to reception staff on our appointments line. 

To arrange an appointment with the treatment room please telephone the appointments line.

To arrange an appointment for the bloods clinic please telephone the appointments line. Appointments for the following week will be released at 8.30am every Thursday morning.

At present the option to book appointments online is not available.

 

Order repeat prescriptions

 

You can either request prescriptions via our online Patient Access portal, or by completing the request slip attached to your prescription and posting this in the locked script request box at the front door of the practice. If you still need to speak to a member of staff regarding a prescription request please phone the surgery and choose the prescription line between 2pm and 5pm on a Monday, Tuesday, Thursday and Friday or between 11am and 12.30pm on a Wednesday.

Please use the request slip from your previous prescription. If you wish you may send a stamped addressed envelope. Please allow two working days to process including consultant outpatient prescriptions NHS or privately.

 

Obtain test results

 

For the results of  test arranged by your GP please contact the surgery on the results line from 2.30pm – 4pm on Monday, Tuesday, Thursday or Friday. Please note that if tests are arranged by a hospital clinic we advise you contact them directly for your results.

 

Inform the practice of a change of address

 

It is important that our contact details for you remain up to date. Please notify staff of a change to your contact details and to notify us of a change of address please download and complete the form attached and give to reception change of address form

 

Make a complaint

 

Our aim is provide the best standard of care possible. There may be times when you feel this has not happened. We hope that, on those occasions, you will use our practice procedure to allow us to investigate and, if necessary, correct any problems you have identified or mistakes we have made.
